New student enrollment requirements Each new family will be required to bring the following items for enrollment:
1. Birth Certificate (hospital or mother's copy) 2. Social Security number (card copy / IRS tax form) 3. Immunization Record and recent physical (TN green or approved white copy) 4. Custodial / Legal / Power of Attorney documents 5. Verification of address in Ruby Major zone
